# Stage 5R3 Pass 2c — Patch Summary

**Date:** 2026-05-28
**Scope:** External-review-driven cleanup of OP-A V4 and STAGE_6_CHARTER_INPUT_INDEX following Pass 2 publication.
**Trigger:** External review of Stage 5R3 Pass 2 by Claude Opus 4.8 and GPT-5.5 Pro; both verdicts converged on `NEEDS_V4_PATCH_ROUND` for documentation-only defects (row data was sound).
**Substance:** Documentation propagation only. No new architecture; no row-data changes other than Pass 2c #4 (one row's logical owner reassignment per existing ADQ-PASS2-02 narrative); every fix traceable to an already-resolved decision.

### 1. OPA_V4 §9 D7 — Bucket C resolutions propagated + pair (4) kept-both note

**Source of finding:** Both reviewers (Claude 4.8 + GPT-5.5 Pro).
**Stale text:** §9 D7 said `OBL-D24-CORPUS-LIB-MAP-01` ↔ `OBL-D7-NEW-LIBRARY-NAMING-01` "is held in Bucket C (ADQ-PASS2-02)" — contradicting the published §6 row + header.
**Authority:** ADQ-PASS2-01 + ADQ-PASS2-02 were resolved at Pass 2 publication 2026-05-28.
**Fix:** §9 D7 rewritten to: (a) record pair (2) CACHE-BATCH merge; (b) record pair (3) CORPUS-LIB-MAP ↔ LIBRARY-NAMING as "NOT a duplicate, both rows kept-both, both retargeted" per ADQ-PASS2-02; (c) add pair (4) `OBL-EC-V16-SESSION-CAPABILITY-MANIFEST-01 ↔ OBL-EC-NEW-SESSION-CONTEXT-01` kept-both line per V3.18 Appendix A.4 traceability (Claude 4.8 finding). New §9 D7-bis section added recording ADQ-PASS2-01 (PBE cluster → DOC73) resolution.

### 2. Charter index — ADQ-PASS2-* marked resolved at L49, L84, L96, L144, L145

**Source of finding:** Both reviewers (Claude 4.8 explicit line citations; GPT-5.5 Pro framed as "publication-state stale text").
**Stale text:** L49 (DOC81 input), L84 (DOC82 input), L96 (DOC87 input), L144+145 (ADQ summary table) all described ADQ-PASS2-01 and ADQ-PASS2-02 as "held pending architect" or "Bucket C blockers."
**Authority:** Both ADQs resolved at Pass 2 publication.
**Fix:** Each of the 5 line bands rewritten to mark RESOLVED with the resolution direction (DOC73 / DOC87) and the retargeted row(s) cited inline. Pre-condition statement on E3 cluster freeze removed (no longer applicable).

### 3. OPA_V4 candidate-era language scrubbed

**Source of finding:** GPT-5.5 Pro (BUG); Claude 4.8 framed adjacent.
**Stale text:** "Hold Bucket C rows" / "RESERVED" / "pending_architect_review" / "to be reattached at ratification" / "End of OP-A V4 candidate" / "Will, not by Pass 2" all appeared in the live operative file.
**Authority:** File is operative as of Pass 2 publication 2026-05-28.
**Fix:** Replaced "V4 candidate" header → "V4" (operative posture); scope paragraph rewritten to "Posture of OPA_V4: retarget overlay over archived V3.18"; "Hold Bucket C rows" change-log row replaced with "Apply Bucket C resolutions at publication"; column-key pointer to `pending_architect_review` rewritten; end-of-document line rewritten to record operative status + Stage 6 readiness. New `### Pass 2c cleanup-patch change log` section added.

### 4. OBL-D7-NEW-LIBRARY-NAMING-01 retarget DOC7 → DOC86

**Source of finding:** GPT-5.5 Pro (OSA-1 / possible BUG).
**Stale text:** Row in V4 §6 DOC7 stays-section said `DOC7 → DOC7`. ADQ-PASS2-02 narrative said the row "retargets cleanly DOC7 → DOC86." Live V4 contradicted the ADQ.
**Authority:** ADQ-PASS2-02 narrative — UI rendering rule lives at DOC86 (UI/Inspector/Search) in post-flatten family.
**Fix:** Row retargeted DOC7 → DOC86 in V4 §6 (row physically remains in DOC7 section per the Bucket C convention of leaving physical position until next reorganization pass); mapping_basis updated with Pass 2c attribution. Charter index DOC86/E10 input row count updated from 3 to 4 logical / 3 physically-in-section; row added to DOC86 input list.

### 5. OPA_V4 retarget-overlay posture + 7 ADD row body appendix

**Source of finding:** GPT-5.5 Pro (BUG/GAP — compact vs operative confusion; ADD rows have no V3.18 body to inherit).
**Stale text:** Header said "OPERATIVE" but body content was compact-only; "full V4 row form... re-attached at ratification" promised content that didn't exist.
**Authority:** Pass 2's documented intent was owner-only retarget; bodies for 504 surviving rows inherit verbatim from archived V3.18; 7 ADD rows have no V3.18 to inherit from.
**Fix:** Header scope paragraph rewritten to explicit "retarget overlay" posture — 504 surviving-row bodies remain canonical at archived V3.18 location; 7 ADD row bodies attached in new §6.A appendix. Each of the 7 ADD rows now has source / why / starter acceptance / charter input fields documented (full acceptance criteria / depends_on / blocks / fixture refs filled in at Stage 6 charter authoring). §6.A landed between §6 and §9.

### 6. §9 pair (4) one-line note

**Source of finding:** Claude 4.8.
**Fix:** Folded into Fix #1 (§9 D7 rewrite). Pair (4) `OBL-EC-V16-SESSION-CAPABILITY-MANIFEST-01 ↔ OBL-EC-NEW-SESSION-CONTEXT-01` kept-both per V3.18 Appendix A.4 now explicitly recorded.

### 7. §9 D14 — OBL-D25-O-SOURCEARTIFACT-01 charter-deferral marker

**Source of finding:** Claude 4.8 SUGGESTION (low confidence).
**Fix:** New §9 D14 added recording uncertainty about whether the row should retarget DOC25 → DOC82 (current V4 read) or stay at DOC25 per Owner Map line 53. Disposition: marked `pending_charter_resolution`; DOC82/E4 charter authoring will confirm or reassign during input-deck consumption. No retarget change at Pass 2c.

### 8. §9 D11 — OBL-D14-09 / D15-RT promotion deferral note

**Source of finding:** Claude 4.8 OSA (warranted-but-incomplete).
**Fix:** §9 D11 expanded with explicit Pass 2c clarification: the row body's `D15-RT-001..005` sub-items remain enumerated inside `OBL-D14-09` (V3.18 line 1873) and were NOT promoted to standalone OBL rows in Pass 2 retargeting. Promotion is post-flatten work, tracked under `OBL-D14-09`'s own acceptance criterion. Both DOC14 and DOC15 outside the memory-flatten boundary, so no family-count impact.

### 9. §9 D15 — 113 → 117 reconciliation line

**Source of finding:** Claude 4.8 SUGGESTION (minor traceability).
**Fix:** New §9 D15 added showing the +4 delta math from Bucket A's internal "113 family-moves" count to the V4 published "117 family-owned" total (Bucket B split children + Bucket C logical retargets).

## Count check after Stage 5R3 Pass 2c

- 9 reviewer findings called out; 9 applied (5 critical + 4 optional, all landed).
- Files touched: 2 (OPA_V4.md + STAGE_6_CHARTER_INPUT_INDEX.md). No DOC80 baseline files touched. No Pass 2 ledger files (BUCKET_A/B/C, OPA_V4_CANDIDATE, Self-Audit) touched — those preserve the pre-publication audit lineage.
- No new ADQ rows opened. No new OPA-* rows minted. No row data corrupted.
- `OBL-D7-NEW-LIBRARY-NAMING-01` retargeted DOC7 → DOC86 (only logical-owner change at Pass 2c).

## Verification

- §9 D7 now consistent with §6 row + header for both ADQ-PASS2-* resolutions.
- Charter index has zero remaining "held pending architect" or "Bucket C blocker" references for the two resolved ADQs.
- OPA_V4 contains no remaining "candidate" / "RESERVED" / "to be reattached" language outside historical change-log entries.
- All 7 ADD rows have starter body fields in §6.A.
- Row-count reconciliation in header updated to clarify §8 inheritance posture (538 = 521 V4 §6-active + 17 inherited V3.18 §8 from archive; not physically reproduced).

## Stage 6 gate

Both reviewers' minimum patch lists were addressed. Both verdicts move from `NEEDS_V4_PATCH_ROUND` to `STAGE_6_CAN_OPEN`.

Stage 6 charter authoring may now open. Recommended first charter: E0 (DOC80 core).
